Case Management

Once enrolled as a Member, you will co-create an individual service plan with a Care Manager based upon the specific needs of your journey. 

 

  • Case management support to obtain necessary documents such as ID’s, birth certificates, social security cards

  • Education support - finding the necessary training for a realistic path forward

  • Employment and career support - credentialing, resume building, interview preparation

  • Support to find and retain housing 

  • Life skill classes on healthy living, relationships, budgeting, and career readiness. Many meet parole and probation requirements.

  • Incentives that acknowledge your achievements on your path of reentry

  • Referrals to outside programs and partners to ensure your needs are met

  • Membership in a community with others who share your experience

 

*Financial support depends upon full enrollment in programming.
